5th Grade Camp Packets – Due Friday, 12/9/22

5th Grade Husky Families,

Please remember to turn in the folders for Camp Seymour your students brought home a week and a half ago, tomorrow, Friday, 12/9/2022, to their teacher.

The required forms are:
– TPS Assumption of Risk/Permission to Participate Form (#1 – Yellow, 1 page)
– TPS Instructional or Reward/Celebration Extended Field Trip Participation Agreement Regarding Release of Liability, Assumption of Risks, and Medical Services Authorization (#2 – Green, 2 pages, double-sided)
-TPS Field Trip Consent Form and Health Questionnaire (#3 – Orange, 1 page, double-sided)
– YMCA Camp Seymour Health and Medical History Forms (#5 – White, 3 Pages)

If you are missing a form or are unable to locate your child’s packet, please contact Kimberly Syverson or Stephanie Modrijan @ [email protected] or (253) 691.5308.

Packets must be returned otherwise students may not be able to attend 5th Grade Camp.
